Characteristics:
Aciclovir is a synthetic nucleoside analogue of deoxyguanosine with a selective effect on herpetic viruses. The mechanism of its effect consists in selective concentration in the virus-infected cell, where it is activated to produce aciclovir triphosphate, which blocks the synthesis of viral DNA.
Indications:
Aciclovir is used in the treatment and prophylaxis of infections caused by the virus Herpes simplex of type I and II in patients with normal immunological response and in immunocompressed patients (HIV infections, bone marrow transplantations), for the treatment of herpetic skin and mucosa infections including primary as well as relapsing genital herpetic infections and for the treatment of serious infections caused by the virus Varicellazoster. The preparation is intended for adults and children over 2 years of age.
Contraindications:
Provirsan® tbl. is contraindicated in patients with hypersensitivity to aciclovir, ganciclovir or some of the auxiliary substances of the preparation. It must not be administered in the period of lactation. There must be very serious reasons for administering aciclovir in the course of pregnancy.
Undesirable effects:
Aciclovir is usually very well tolerated. In some patients skin rashes may occur, which disappear after discontinuation of therapy. In some cases, aciclovir therapy may be associated with reversible neurological complaints.
Interactions:
Probenecid administration results in prolongation of the biological half-life of aciclovir. Concomitant administration of nephrotoxic drugs with aciclovir increases the risk of the impairment of the renal function.
Caution:
In the old age, total aciclovir clearance decreases parallelly with creatinine clearance, and in the elderly patients it is therefore necessary to pay attention to sufficient hydration.
Dosage:
It is necessary to start therapy as soon as the signs appear. In Herpes simplex infection in adults and children over 2 years of age, 1 tablet (200 mg) is administered 5 times a day for a period of 5 days in 4-hour intervals; one night dose is skipped. In serious infections, treatment can be prolonged. In patients after bone marrow transplantations or in patients with a disorder in intestinal absorption, the single dose can be doubled to 2 tablets (400 mg). The prophylactic dose is usually 1 tablet (200 mg) 4 times a day after 6 hours. In Herpes zoster infection, adults take 4 tablets (800 mg) 5 times a day after 4 hours and skip the night dose for a period of 7 to 10 days. In Varicella infection, adults take 4 tablets (800 mg) 4 times a day after 6 hours for a period of 5 days, children over 2 years of age are administered 20 mg/kg 4 times a day for a period of 5 days, i.e., e.g., when the child’s body weight is 20 kg, it is 2 tablets (400 mg) 4 times a day. Children weighing more than 40 kg receive the same dose as adults. Tablets should be sufficiently washed down. In children under 3 years of age, tablets should be crushed or dissolved in a glass of water.
